Email: eileen@counsellingtoday.co.uk

Phone: 07930564159

This agreement sets out to display the professional therapeutic framework and to receive your informed consent.

Session Arrangements

At the start of our work together we will mutually agree a regular day and time for your therapy sessions.

Sessions are of 50 minutes duration.

If you are unable to attend an appointment, please phone, text or email giving as much notice as possible.

I have a 36 hour cancellation policy when the full fee will be charged.

The agreed fee of £90.00 to be paid on the day of the session by debit/credit card or BACS transfer to the following;

Name: Eileen D

Sort code: 07 02 46

Account: 14370970

Confidentiality

All therapy sessions will be conducted in confidence, however, if it appears you are at risk of harming yourself or others, we will explore this initially and I may contact your GP if necessary.

When used as a reference, your name will appear on my bank statement when payments are made by BACS. Using the card reader may include your name on statements from the provider, all of which are contained to a paperless online trail.

Boundaries

The boundaries of the professional relationship do not permit interactions on social media. Additional boundaries will be discussed and established in our first session together including but not limited to unplanned face-to-face interactions. In typical practise when confronted with this situation, I will not acknowledge you unless you choose to acknowledge me in order to respect and protect confidentiality.

If a conflict of interest arises, such as a dual relationship, a discussion to consider if continuing therapy is appropriate will take place.

Delivering Therapy by Electronic Means

Online therapy will be delivered through a suitable platform accessible to both client and therapist. Security and call quality cannot be guaranteed.

Please use a confidential space where you will not be overheard when attending online therapy.

Record Keeping

I take brief hand-written anonymised notes which are kept in a locked cabinet separate from any identifiable information, you may have access to the notes on request. At the end of therapy records will be kept for seven years unless agreed otherwise.

I am registered with the Information Commissioner – Ref: ZA407436

Ending Therapy

A planned ending is optimal in therapy, which can be subject to review. However, if you decide that you no longer wish to continue therapy please make this known so that a single ending session can be offered.

In the event of sudden incapacity, a professional executor will take responsibility to contact you. In such case identity would be disclosed, but executors are bound by the same professional rules of confidentiality.

Supervision

As a requirement of the professional bodies of which I am a member, I receive regular professional supervision. Any material discussed with my supervisor is anonymous and remains confidential.

Code of Ethics

As a member of the British Association for Counselling and Psychotherapy (BACP) and the College of Sexual and Relationship Therapy (COSRT), I adhere to their ethical framework for good practice and to the professional conduct procedure.
